Dr. Boulware grew up in Irving, Texas and attended The University of Texas at San Antonio Medical School. He completed his residency in family practice at Bayfront Hospital, and has been serving patients in both family practice as well as minor emergency procedures for over 20 years.
Dr. Boulware and his family moved from Tampa, Florida to Madison in 2020.
