Application Requirements

• Completed application form
• Official transcript from high school or college
• Proof of enrollment/acceptance letter from nursing program (RN accredited program)
• Personal essay (500-1,000 words)
• Three letters of recommendations
• Financial aid documentation (if applicable)
• Resume
• Career goals statement (300-500 words)
• Signed declaration & agreement
• Completed interview with MMC Scholarship Committee during the selection process
