Overview

Morgan Medical Center (“MMC,” “we,” “our,” or “us”) provides the MMC MyHealth Patient Portal (“Portal”), a secure online platform powered by Meditech Expanse that allows patients to view portions of their health record, communicate with their care team, and manage healthcare information. By creating or accessing a Portal account, you agree to these Terms of Use and this Privacy Policy (collectively, the “Agreement”). If you do not agree, you may not establish a patient portal connection.

Eligibility and Access

You must be at least 13 years old to establish a Portal account. By registering, you represent that the information you provide is accurate and complete. You are responsible for maintaining the confidentiality of your username and password and for all activities that occur under your account. If you suspect that your credentials have been compromised, promptly reset your password and notify us at [email protected] or 706-342-1667.

Proxy and Authorized Access

If you are legally authorized to view another person’s health information (for example, as a parent, legal guardian, or healthcare proxy), you may be granted access subject to applicable laws. Certain sensitive information, such as reproductive health, mental health, or substance use information, may not be viewable based on federal or state restrictions.

You may also grant proxy access to others. You are responsible for the actions of any proxy you authorize. You may modify or revoke proxy access at any time.

Use of the Portal

The Portal is intended solely for your personal, non-commercial use to access health information and communicate with your care team. You agree not to use the Portal for unlawful purposes or in any way that could damage or impair the Portal or interfere with others’ use. You agree not to attempt to gain unauthorized access to any systems, accounts, or data or to upload or transmit harmful code, spam, or inappropriate content.

Privacy and Security

Your use of the Portal is subject to this Privacy Policy and the MMC Notice of Privacy Practices, which explains how we safeguard and use your protected health information (PHI) under the Health Insurance Portability and Accountability Act (HIPAA) and applicable Georgia law.

We implement technical, administrative, and physical safeguards to protect your PHI, including encryption, secure authentication, and audit controls. While we take reasonable measures to protect your information, you acknowledge that no online platform can guarantee absolute security.

Information Collected and How It Is Used

The Portal may collect information from you in the following ways: From you directly when you register, communicate with your provider, or upload information, we collect details such as your name, contact information, and health data. From system logs and analytics, we may collect IP addresses, device identifiers, and usage data to maintain system security and performance. Cookies and similar technologies may be used to maintain your session and enhance user experience. You may disable cookies, but doing so may limit Portal functionality. We use this information to operate the Portal, deliver services, and communicate with you about your care.

Sharing and Disclosure of Information

We do not sell your personal information. We may share your information as follows: With Care Providers and Staff to support your treatment and care coordination. With Vendors and Service Providers including Meditech Expanse and other partners who provide technical, hosting, or operational support under business associate agreements. When Required by Law to comply with legal obligations, court orders, or regulatory requests. For Public Health or Safety when necessary to prevent or lessen serious threats to health or safety.

Your Rights

You have the right to: Access and review portions of your medical record available through the Portal. Request amendments or corrections to your health information through established MMC procedures. Request restrictions or confidential communications permitted under HIPAA. Obtain a record of certain disclosures of your PHI. Patient Responsibilities

By using the Portal, you agree to: Use the Portal only for yourself or authorized individuals. Communicate respectfully and appropriately with staff. Review results and messages in a timely manner. Notify MMC of any inaccurate or outdated information.

The Portal is not intended for emergency use. If you are experiencing a medical emergency, call 911 or visit the nearest emergency department.

Limitation of Liability and Disclaimer

The Portal and its content are provided on an “AS IS” and “AS AVAILABLE” basis without warranties of any kind. MMC and its suppliers make no representations or warranties regarding reliability, accuracy, or availability. To the maximum extent permitted by law, MMC and its affiliates disclaim all liability for any damages arising out of your use of the Portal. Your sole remedy is to discontinue use.

Termination

MMC may suspend or terminate your Portal access at any time, with or without notice, for any reason including misuse or inactivity. You may terminate your own access at any time by contacting MMC Health Information Management.

Updates to This Agreement

We may revise this Agreement from time to time to reflect updates in technology, law, or our practices. Material changes will be communicated through the Portal. Your continued use of the Portal after such updates constitutes acceptance of the revised Agreement.
